1. Go to the Create RFA Page From the Manage Users Page,

2. Add RFA Information

3. Enter Employee Information

4. Enter Requesting Physician Information

5. Enter Requested Treatment Information

Tips & Hints! The fields for an entry will appear once the button is clicked. You can have multiple entries by clicking the new buttons. Click on the X to remove an entry.

6. Enter Claims Administrator Information

7. Enter URO Response Information
